Unlocking a cell phone - where do the codes come from?

Guys-

 

Was just working on the third-party unlocking options in the How to Unlock a Cell Phone wiki, and realized there is one thing I don't know: where do thirty party unlocking companies get their codes from? Since you pay for them, I have to imagine that they pay somebody else, but who?? The carrier? The manufacturer?

I think they pay for the carrier. When my mobile locked with T-Mobile i got the code from http://www.onlinegsmunlock.com/
